How We Help Clients
The primary way we help clients in DREAMS is by providing one to one care. The reasons we provide one to one care vary depending on the client's needs, but the main aim is to assist the client to actively participate in their community. In order to provide quality programming a few things need to happen when a new client becomes part of the DREAMS team. First, we need to sit down with the client and their stakeholders to get a better picutre of who the individual is. We do this by asking some important questions about the skills, abilities, needs and preferences the individual may have. We want to be sure we know as much as we can about the individual in order to best plan their program activities. Through discussions with the client and stakeholders, observations that have been made, and the client's history we determine the needs of each client and gain a better understanding of how an individual would benefit from the sort of support that DREAMS provides. To be successful in meeting the needs of the clinets we develop individual support plans, behavior supports, and individual program plans. Once these important plans are developed, the staff have clear concrete tools to provide the best possible support to the individuals.
